According to the American Heart Association, nearly half (46%) of adults have high blood pressure in this country, and it's a major risk factor for chronic illness.
That's why the YMCA designed our signature Blood Pressure Self-Monitoring program. Our evidence-based model features personalized support for developing a blood pressure self-monitoring routine, tips for maintaining cardiovascular health and nutrition education!
The simple process of checking and recording your blood pressure at least twice a month over a period of four months has been shown to lower blood pressure in some people with hypertension. In the Blood Pressure Self-Monitoring program, we combine regular checks with support from trained Healthy Heart Ambassadors, and nutrition education to give participants the tools they need to improve their wellbeing.
Through this four-month program, and with the support from a trained Healthy Heart Ambassador, participants will aim to:
• Measure and record their blood pressure at home at least two times per month, gaining increased awareness of triggers that elevate blood pressure
• Attend two personalized consultations per month with a Healthy Heart Ambassador
• Attend monthly Nutrition Education Seminars
To participate in the program, participants must be:
• Be at least 18 years old
• Be diagnosed with high blood pressure
• Not have experienced a recent cardiac event
• Not have atrial fibrillation or other arrhythmias
• Not be at risk for lymphedema
Participants pay a one-time fee that covers the cost of the program. Each person will receive a blood pressure cuff to keep and use at home.
